Population Health Project Slide Presentation
The health project will be comprised of an assessment of a population, a teaching related intervention based on the assessment, specific nursing theory you choose, and an evaluation plan for the process.
You will not need to carry out the teaching activity but this project will have to be thoroughly planned. As an advanced nurse teaching is an important tenant of nursing practice.
Each student will pick a topic. Please see the grading criteria for the presentation to determine the minimal content that must be included within the presentation.
1) Assessment of population (community) needs on the selected topic, based on literature review, describe factors that contribute to your disease;
2) Theoretical Framework: Selection and application of a nursing, or borrowed, theory with major concepts applicable to the project (use one of the following theoretical frameworks- Pender’s Health Promotion Model, The Health Belief Model, or Social Ecological Model).If you would like to use a different framework please send me an email and you must receive approval prior to using in your presentation.
3) Major policy, goals and objectives of the project identified, and a health education plan with sound teaching principles, with interventions related to primary, secondary, and tertiary care. Also, identify 2 accessible resources that would provide educational or some type of benefit that would be specific for your topic.
4) Evaluation plan with specific instruments/measures identified. How would you evaluate that your teaching was a success or measure what needed modification. (e.g., pre-test/post-test; evaluation form, number attending and retained over time; biometric change- change in weight, blood glucose level, etc.),
5) Conclusion, integrating the information discussed above, and determining implications for on-going nursing practice and future research. How would you recommend that implementation occur in the future?
These recommendations are provided as a guide. For example, the needs assessment must be completed before the rest of the project can go forward.

You can copy and paste it in your browser and change the state, to locate your information. You will identify a community, and propose a population health project, based on the needs assessment. This project will obtain its structure based on standing health policy, and goals and objectives of HealthyPeople 2030. Specific goals and objectives will be formed for the project, with a teaching plan, and an evaluation plan that is based on the planned activities. Finally, a discussion about the major findings and parts of the proposed population health project will be included in the conclusion, along with identified implications for nursing practice and opportunities for future research.
